Release of Liability and PassCash Agreement
New information as of November 8, 2011 regarding Liability Releases for 2012 passholders
Passes have now all been shipped. The fulfillment letter contains swipe numbers for passes we were unable to ship due to missing documents. Passholders who have not completed the necessary documents should use the form below to electronically complete either a Liability Release and/or a Pass Cash Agreement (if needed). Please allow a few moments for the form to appear.
If you are concerned or if you have recently changed your email address or street address, please call 800.MAMMOTH and a friendly agent will check the status for you.
Important information about your 2011/2012 pass order.
Liability Release Form.
Mammoth will not issue any Season Pass without first obtaining a validly signed Liability Release. This form is required for each Pass purchased. Forms for children under 18 are required and must include a signature by a parent/ legal guardian. You or the purchaser of your pass should have already received an electronically signable document via email.
PassCash Agreement.
Mammoth will not issue your new pass without a validly signed PassCash Agreement if you have a balance on your pass from previous seasons OR if you purchased bonus pass cash last spring. Forms for children under 18 are required and must include a signature by a parent or legal guardian. You or the purchaser of your pass should have already received an electronically signable document via email if we are in need of this document.
Please note that we have modified the terms of the PassCash Program Agreement as follows:"Beginning June 1, 2011, you will not be able to use PassCash to purchase a season's pass, Mammoth Black or gift cards. To view the entire PassCash Agreement, please click here.
Electronic Signature Information
Mammoth is now partnered with

You may be required to provide a valid email address and respond to an email sent to this address in order to complete the signature process.
WARNING: Under penalty of perjury, you must only use this functionality to sign for yourself and to sign for your own minor children or for children for whom you are a legal guardian.
All passholders should use this option.
You should have already received an email with directions. If not, please call 800.MAMMOTH as we may have incomplete or incorrect contact information for you.
Should you choose not to take advantage of the ease of electronic signatures, you will need to sign the document(s) in person and pick up your pass at a primary ticket window (Main Lodge, Canyon Lodge, Mountain Center or Little Eagle)
